Are You at Risk for a Heart Attack or Stroke?
If you answer "yes" to two or more of the following questions, the American Heart Association recommends you talk to a healthcare provider for further assessment of your cardiovascular health.

Are you a man aged 45 or older?
Yes  No

Are you a woman aged 55 or older?
Yes  No

Do you have a father or a brother who had a heart attack before the age of 55—or a mother or sister who had one before the age of 65?
Yes  No

Do you have a close blood relative who had a stroke?
Yes  No

Do you smoke—or do you live or work with people who smoke every day?
Yes  No

Do you have a high-stress job or lifestyle?
Yes  No

Is your total cholesterol level 240 or higher?
Yes  No

Is your HDL (or "good cholesterol") level less than 35?
Yes  No

Is your blood pressure 140/90 or higher? (Or have you been told that your blood pressure is too high?)
Yes  No

Do you get less than a total of 30 minutes of physical activity on most days?
Yes  No

Are you 20 pounds or more overweight for your height and build?
Yes  No

Do you have diabetes or a fasting blood sugar of 126 or higher, or do you need medicine to control your blood sugar?
Yes  No

Do you have coronary heart disease or have you ever had a heart attack?
Yes  No

Have you been told that you have carotid artery disease, have you ever had a stroke or TIA, or do you have a disease of the leg arteries?
Yes  No
