Are you 20 pounds or more overweight for your height and build? Yes No
Do you have diabetes or a fasting blood sugar of 126 or higher, or do you need medicine to control your blood sugar? Yes No

Have you been told that you have carotid artery disease, have you ever had a stroke or TIA, or do you have a disease of the leg arteries? Yes No
